Please include this form with your check to ensure we have all the information required by the NC Board of Elections.
Under North Carolina law we may only accept donations from individuals, not businesses. Gifts of $50 or more must be made by check or charge and include your name, mailing address, job title or profession, and name of employer or employer’s field. Limit $4000 per campaign cycle.
